  1. go to https://ungm.in-tend.co.uk/wipo/aspx/ProjectManage/255 and log-in (or register) to review the documents therein,
  2. download Annex B – Price Schedule and complete it offline, and
  3. using the same System, submit the completed Annex B on or before the deadline of December 14, 2015 at 11h00.
