Title of the assignment

Installation, commissioning and maintenance of Central Medical Gas Pipeline System by manifold in 60 selected Upazila health complexes of Bangladesh to improve oxygen supply for hypoxemia management of COVID patients

Purpose

Hiring engineering technical company qualified to provide site assessment, design and site supervision services, including safety compliance and quality assurance for the Installation, commissioning and maintenance Central Medical Gas Pipeline System by manifold in 60 selected Upazila health complexes of Bangladesh to improve oxygen supply for management of COVID patients.

1. Background

WHO declared Covid 19 a worldwide pandemic on 11th March 2020[1] while Bangladesh reported its’ first positive case on 8th March. Till 11th January 2022, 1.6 million confirmed cases reported of COVID-19 in Bangladesh, of whom 28,105 died.

Till date all districts of Bangladesh have reported confirmed Covid-19 cases and the country has been categorized as Cluster of cases with Dhaka City being the worst affected followed by other divisions.  These districts along with their adjacent districts are now regarded as the epicenters of the COVID in Bangladesh. Given the unprecedented nature of SARS-COV-2, the virus that causes COVID-9 disease, any district might become the new epicenter and pose a great risk for overwhelming the health facilities/hospitals with COVID cases.

While most people with COVID-19 develop only mild illness, approximately 14% develop severe disease that requires hospitalization and oxygen support, and 5% require admission to an intensive care unit [2]. Most of the severe and critical COVID cases would require high flow oxygen support and the requirement of oxygen for the projected number of cases at Upazila, district, divisional and tertiary level hospitals may far exceed the current capacity. During the second wave, it was observed that the burgeoning number of COVID cases were over stretched the case management capacity of the designated hospitals which have been further accentuated by the inadequate oxygen supply at all level of hospitals. In Bangladesh all the upazilla and district level hospitals don’t have oxygen generation capacity and largely depends on supplied oxygen cylinders. Even though some of district hospitals and the medical college hospitals and tertiary level hospitals have either Liquid Medical Oxygen (Vacuum Insulated Evaporator-VIE) system or small numbers of PSA (Pressure Swing Adsorption) system, however those are not sufficient to cater the increasing demand of oxygen with high flow for managing the COVID cases as per national Case Management guidelines specially during surge. Thus for effectively managing the COVID cases, all hospitals at district, divisional and tertiary level managing large numbers of severe and critical cases should have central oxygen management system with surge capacity through oxygen generation system (PSA or LMO) and high burdened Upazila health complexes should have central oxygen management system with surge capacity through Manifold system backed-up by adequate oxygen distribution through pipelines and cylinders. Meanwhile Oxygen concentrators are only suitable to manage severe cases who require <10LPM oxygen and may be considered mainly for the remaining Upazila Health Complexes and other facilities as per requirement.

In alignment with the National Preparedness and response Plan and as one of the strategic priority for COVID case management, DGHS has already assessed the facilities in terms of oxygen capacity and developed a plan for further strengthening the oxygen supply system in all designated hospitals including the Upazila health complexes who will be managing COVID/Non-COVID cases.

To manage the highload COVID 19 cases, DGHS has already established LMO system in 142 designated facilities including Medical colleges and district hospitals. In the context of current situation and urgency observed during the second wave, DGHS assesses the case load in UHC during surge and prepared a list of Upazila health complexes for installation of oxygen manifold system with extended outlets to surge the oxygen supply capacity of all hospitals within shortest possible time period.

UNICEF steered an analysis and projection of oxygen requirement by different level of facilities and conducted policy advocacy for immediate strengthen of oxygen capacity in all designated hospitals, which prompted the DGHS, MOHFW and other partners to take affirmative actions in this regard, which also positioned UNICEF as a trusted development partner to manage and implement Oxygen supply system through a third party technical agency.

 

2. Objectives, Purpose and Expected Results

Objectives and Purpose of Assignment – The objective of this project aims to improve the capacity of the health facilities at upazila level for management of COVID-19 cases through upgradation of Oxygen supply system as per national management protocol hence reducing the patient load in district hospitals. Within the framework of Bangladesh National Preparedness and Response Plan (BNPRP), UNICEF is looking at hiring an engineering technical company qualified to provide establishment of manifold system with design and site supervision services, including safety compliance and quality assurance for the Installation, commissioning and maintenance of oxygen manifold system in selected 60 Upazila health complexes of Bangladesh to improve oxygen supply for management of COVID patients.

Specific services under this assignment are as follows:

  • Physical structural analysis for selected UHC to have clear understanding of the renovation or construction requirement.
  • Design for Medical Gas Pipe Line as per standard (HTM-2022/NFPA-99)
  • Detail Technical Specifications in compliance with HTM-2022/NFPA-99;
  • Detail Quality assurance, Safety and regulation compliance plan;
  • Installation, Testing & Commissioning of Central Medical Gas Pipeline System by manifold
  • Site supervision and quality assurance
  • Renovation work for manifold room based on site assessment report.
  • Preventive maintenance, trouble shooting, repair-maintenance (Detail and clear operational and maintenance plan and capacity building)
  • Detail training plan for Hospital stuff including On Site training
  • Operational plan including shared responsibility with hospitals in terms of oxygen storage, replenishment and reserve supply etc.
  • Detail plan for Oxygen refilling, supply and capacity of the vendor (Vendor needs to submit evidence of their sufficient capacity to provide additional oxygen to 60 facilities in the proposal)
  • Detail coordination plan with MOHFW, DGHS, HED, PWD, NEMEMW and UNICEF
  • Equipment & Pipeline Warranty Period -1 Year (Minimum)

 

3. Description of Assignment

 

Scope of Work – The technical agency will coordinate with DGHS, NEMMU, HED, UNICEF and other concerned agencies for the installation of Central Medical Gas Pipeline System as per the list underneath as per the guidelines and specifications set by the DGHS/MOHFW.

 

Description of tasks:

 

  1. Initial Site assessment, Physical structural analysis; Detail Technical Specifications in compliance with MOHFW’s guidelines, detail design (MGPS); Prepare detail Project Implementation Plan along with detail site-based work plan and Team mobilization plan and BOQ;
  2. Prepare Safety and regulatory compliance plan and ensure its’ compliance in accordance with the government’s regulatory requirement, relevant codes and as per the standard industrial practices and standards;
  3. Carry-out physical works for the installation Central Medical Gas Pipeline System by manifold  in accordance with all applicable local codes, standards, rules, regulations, and laws including, but not limited to, the codes and standards referenced in the detail agreed Specification (to be annexed);
  4. Installation, Testing & Commissioning of Central Medical Gas Pipeline System by manifold  in total 60 UHC Hospitals as per the list provided underneath and in accordance with all applicable local codes, standards, rules, regulations, and laws including, but not limited to, the codes and standards referenced in the detail agreed Specification (to be annexed);
  5. Develop detail operations, maintenance and site supervision and quality assurance plan which includes but not limited to adequate supply chain with measure and control devices for periodic check for purity, pressure and consistency of oxygen; after completing the MGPS work all test report (Leak Test, Cross Connectivity & Purging) should be signed by authorised biomedical engineer.
  6. Capacity building of the Individual hospital staff on user operation, maintenance including refresher and onsite training plan which should include
  • Capacity building of managerial personnel, Bidder should submit a training plan  
  • Capacity building of user/Technician, Bidder should be submitted a hands on training plan

  • Supply Status Inspection. (Only perform this step if the manifold is used as an emergency backup supply).
  • Check that all cylinders are properly stored/secured and all batch labels are correct and in date.

** Filling of the oxygen will be done by the supplier during the installation of the Central Medical Gas Pipeline System in designated UHCs. After that Ministry of Health will ensure the oxygen supply for refilling of oxygen cylinders for manifold as rental basis or any other provisions as per existing mechanism followed by the hospitals under the MOHFW.

It may be noted that considering that this project is critical for the COVID19 response the implementation of Central Medical Gas Pipeline System by manifold at the shortest possible time in the hospitals referred below would be the key factor.

5. Reporting requirements

  2. Monthly progress update report on operations, Site supervision and quality assurance visits and maintenance of the oxygen supply system including capacity building
  3. Two reports on completion of Installation, physical works Testing & Commissioning of oxygen supply system in 60 hospitals (one after 70% completion, second one after 100% completion)
  4. Submission of final project completion report.
  5. Interim report, presentation on updates as required by UNICEF or DGHS

 

6. Payment Schedule

  • First payment- 20%:
  • Upon submission 1st deliverable of inception report which includes Initial Site assessment, physical structural analysis; Building design (including mechanical, electrical, plumbing and safety); Permitting; prepare BOQ; team mobilization plan and Project Implementation Plan along with quarterly work plan;

 

  • Second payment- 30%
  • Upon submission of an activity report on completion of at least 70% Installation, Testing & Commissioning of oxygen supply system in 60 upazila health complexes;
    • 5% of the  above payment will be retained and will be released 6 months after successful operation of the system.

 

  • Third payment – 30%
  • Upon submission an activity report on 100% completion of Installation, Testing & Commissioning of oxygen supply system in 60 upazila health complexes;
    • 5% of the  above payment will be retained and will be released 6 months after successful operation of the system.

 

  • Fourth payment -20%
  • Submission of final project completion report to be submitted with second quarterly report;

 

7. Qualification requirement of the company/institution/organization

 

  1. Qualified agency or institute should have minimum 5 years of experience in establishing large scale of Medical Gas Pipeline System (MGPS) for Upazila, district and tertiary level hospitals;
  2. The agency shall have successfully completed minimum 01 (one) number of similar works (Supply and installation of medical gas pipeline) within last 5 (five) years in Government/Semi Government/Autonomous organization of Bangladesh with a value of at least 50% to 75% of offered project value in a single contract.
  3. Should have proven track record and expert team for installation of large scale oxygen plant/ Liquid Medical Oxygen system/ Manifold System, distribution system, commissioning, quality assurance, safety compliance and maintenance; 
  4. Should have a local setup/representative with adequate work experience in establishing/ facilitating large scale Oxygen supply system for medical hospitals.
  5. The Bidder must enclose VAT registration certificate, valid TIN certificate, Up to date trade license, Audit report for the last 2 (Two) years, Government of Bangladesh.
  6. The Bidder shall be the sole local agent / distributor of the proposed brand equipment. In this regard documents shall have to be submitted along with the tender.
  7. Similar work experiences with the Ministry of Health, Government of Bangladesh would be an added advantage.

Supplying of medical gas and vacuum outlets conforming following standards. Terminal units shall be gas specific only to accept correct medical gas probe. For positive pressure gas services, the outlet should be equipped with a primary and secondary check valve and the secondary check valve should be rated at a minimum of 1379 kpa in the event the primary check valve is removed for maintenance. Outlet bodies should be gas specific by indexing each gas service to a gas specific dual pin indexing arrangement on the respective identification module. Gas specific components within a terminal unit shall be pin indexed to ensure that a correct gas specific assemble is achieved. Each terminal unit shall incorporate a maintenance valve, designed to shut off the gas flow when the terminal unit is removed for maintenance purposes. The units shall be capable of single-handed insertion and removal / push type. A color coded front plate should be used for ease of gas identification and aesthetic appeal. A one-piece fascia plate should frame the outlet. The units should be suitable for concealed installation and should be complete in all respect including chromed fascia. First fix assembly, second fix assembly, fascia plates, fixing screws etc. distance between outlets must conform to recommended minimum and the facial plate shall be sized accordingly. All outlets shall be 360-degree swivel inlet pipe for easy installation. All outlets shall be cleaned and degreased for medical gas service, factory assembled, tested. Supplying of medical gas line ball valve, assembles shall be constructed of a nickel plated brass body, approved type seal and a chrome plated ball. The valve shall be operated by a manual operating level selected through 90 degrees. Valves shall be operated by a lever-type handle requiring only quarter turn from a fully open position to a fully closed position. Ball valves shall be equipped with washed and degreased copper pipe stub extension at both the inlet and the outlet sides of the valve port to facilitate installation. Valves should be designed in such a manner that it can be swung-out during installation so as to prevent damage due to heat transfer during the brazing operation. Each valve assembly shall be washed and degreased for medical gas service. The Line Ball valve should have dual gauge port with includes a gauge meter, where easily can see the pressure manually.

 

Pipe stub extension shall be capped at both ends. The valve shall be supplied in sealed plastic bag to prevent contamination prior to installation.

 

All ball valves shall provide a full bore flowing be properly cleaned and tested. The valves shall be complete with necessary adapter, bonded seals, sockets and other accessories.

MANIFOLD CONTROL SYSTEM

Supplying & Installation of manifold control system (MCS) shall supply medical gas from both left and right hand manifold bank. The operation and performance criteria shall fully satisfy the requirements of following standard. The MCS shall operate at a maximum inlet pressure of 20,000 kpa and provide a distribution system pressure of 345 kpa-800 kpa. Either the left or right hand manifold bank may be designated "duty" or the MCS shall manual type change over to supply the distribution system from the "stand by" bank when pressure in the duty bank falls to 800-2000 kpa.

 

The pressure control cabinet shall contain dual pressure regulators and switches that assure an uninterrupted, regulated flow of gas

To the pipeline. The cabinet shall have a digital pressure display and analog three pressure gauges, one for each supply side and one for the pipeline delivery pressure. A group of indicator lights shall be used to indicate the status of the left and right cylinder banks. 

 

High pressure gauges shall indicate gas contents in the respective manifold cylinder bank and a supply pressure gauge shall indicate -distribution pressure. The supply system shall be protected by 25 micron fillers with replaceable elements and a pair of matching pressure relief valves shall be provided. High and low line pressure switch shall sense distribution system pressure and operate both local and remote alarm indications.

Local indications shall be utilized colored long life high intensity LED's on the control panel to indicate the bank in use for high and low pressure supply conditions, Selector switches shall be provided to activate a manual change over and operate test circuits to prove LED integrity. Volt free terminal shall be provided for connection to a remote alarm system. Design of manifold control system shall be such that replacement of any component in the control and monitoring system in possible un interruption of supply.

 

The header bars shall be modular in design, allowing for future expansion. Each inlet port shall be equipped with an individual check valve and each brass header bar shall include a master shutoff valve to shut off the gas in emergency situations. Cylinder connections shall have a stainless steel mesh covering and be flexible to facilitate cylinder alignment with the header bar and include gas specific CGA fittings on each end.

Piping regulators and control switchover components shall be cleaned for oxygen service. Oxygen manifolds shall be equipped with copper pigtails.

Supplying of area alarm which shall be microprocessor based with individual microprocessors on each display and sensor board. The sensors should be capable of local (in box) or remote mounting. Each sensor and display unit shall be gas specific, with an error message display for an incorrect connection.

 

The area alarms shall be of modern construction and field expandable with the addition of extra modules. Up to six services (LED) / eight services (LCD) can be accommodated as per standard box. The alarm system shall have an audible warning which shall operate simultaneously with any failure indication and a mute facility shall be provided. Each specific service shall be provided with an LED digital / LCD read-out comprising of 0-250 psi [0-1724 kPa] for pressure and 0-30"Hg [-100-0 kPa] for vacuum. The digital read-out should provide a constant indication of each service being measured. Each gas service shall be displayed by colored LEDs, to show normal, low and high pressure conditions and also have to be digitally numeric readout. Failure indicators shall be displayed by flashing light and the normal indications shall be steady. A bar graph trend indicator shall be provided for each service indicating a green "NORMAL", yellow "CAUTION" and a red "HIGH" or "LOW" alarm condition. Under normal operation the bar graph display should move up and down in the "GREEN" range depending on service usage. If an alarm occurs, the "RED" alarm light shall flash and the audible alarm shall sound. Pushing the "ALARM MUTE" button shall cancel the audible alarm, but the unit shall remain in the alarm condition until the problem is rectified. In addition, "TEST" & "ALARM MUTE" buttons shall be easily accessible to operate and test the unit

 

The system shall satisfy the following standard. The gas failure alarm system shall be installed at the location shown in the drawing with necessary service failure sensors compatible with the gas alarm system, sensor cable and shall be complete with all necessary accessories.
